Summer Camp Forms and Information
The forms below have been approved by University Counsel, Academic Council and Cabinet for use by all UNCW offices and/or departments who bring minors to campus for camps/programs or other camp-like events. The office/department responsible for the camp/program must have a set of forms completed and signed by the participant's parent or legal guardian.
Set A - Long Forms
These forms are used when the participants are engaged in outdoor and/or physical activity or overnight residential camps.
