Student obtains form from advisor or graduate coordinator (the same form is used for undergraduate DIS). The instructor completes information, including course title, up to 32 characters (this information will appear on transcript); student secures signatures and all copies of the form is forwarded to the Graduate School. The information must be typed or printed legibly. After all signatures are secured, the Graduate School approves the DIS and distributes copies as required. This form should be completed and on file in the Registrar’s Office by the end of drop/add.
