Must be signed by the student. An additional copy should be printed for the Cashier's Office. |
| Pass/Fail Option | Must be signed by the Instructor and Dean. |
| Request to Allow Class Time Conflict | Must be signed by the student, both course Instructors, and Dean. |
| Undergraduate Request to Enroll in Graduate Course | Must be signed by the student, Instructor,Department Chair, Academic Dean, and Graduate School. |
| Permission to enroll in Departmental Honors forms should be requested from the Honors Scholars Program Office. | |
| Transient Study | Must be signed by the Student and Department Chair. (Residency requirement waivers must also be signed by the Dean.) |
