PRIOR TO THE STUDENT ARRIVING IN CLASS

Each student requesting accommodations must register with Disability Services and complete two key pieces--the Background Information & the Request for Accommodation Form--prior to any accommodations being authorized.

• Sample Background Form: Completed once per academic year
  
• Sample Request for Accommodation: Completed each semester in order to receive accommodation letters to distribute to faculty.

 

WHAT A STUDENT WILL BRING TO HIS/HER FACULTY

A student requiring accommodations due to a verified disability will present his/her instructors with Accommodation Letters created by Disability Services. These letters will indicate when the letter was produced and what accommodations are approved by DS.
