The SHC Medical Records Department is responsible for receiving, filing, and securing your immunization records, medical records, and telephone/fax messages regarding your care. All students presenting for health care will have a medical record on file. Copies of Medical Records If you wish to release your medical record to anyone, you must first sign a release form, which is available at the SHC. Allow a minimum of 24 hours for duplicating and mailing of medical records. Medical records copied for referrals to other health care providers are free. A fee is charge for copies made for any other purpose. Medical History Report / Immunization Form Every student is required to complete the "Immunization & Medical History Form". The form is included in the information sent to entering students from the Admissions Office. The form can also be downloaded via the link below or picked up at the SHC. You can avoid long lines and delays in registration by sending your completed form to the Student Health Center prior to orientation, or as early as possible. Forms can be mailed to: UNCW Student Health Center, 601 S. College Rd., Wilmington, NC 28403. Forms can be faxed to the SHC at 910.962.4130. North Carolina law (General Statute 130A 152-157) requires that all students entering college must present a "Certificate of Immunization" within thirty days of registration. By NC law, you will be withdrawn from the University if you do not provide documentation of the required immunizations prior to the deadline. You must provide proof that you have received the immunizations that are required by law. CAREFUL ATTENTION SHOULD BE PAID TO THE INSTRUCTIONS ON THE IMMUNIZATION & MEDICAL HISTORY FORM. STUDENTS WILL BE NOTIFIED OFANY INCOMPLETE IMMUNIZATION & MEDICAL HISTORY REQUIREMENTS. DOCUMENTATION FULFILLING THE REQUIREMENTS MUST BE PROVIDED PRIOR TO ATTENDING CLASSES.
